Journey Clinical BHI Consent

As part of your ongoing care at Journey Clinical, you can join our Behavioral Health Integration (BHI) program. Between your visits, someone on your care team will check in to see how you’re doing, coordinate with your therapist or other providers, and update your care plan as needed. This usually happens by phone, by message or through the patient portal, and it’s in addition to your regular appointments.

Your consent

By checking “I Agree” in the Journey Clinical patient portal, you consent to join the Behavioral Health Integration program, and you confirm that you have read and understood this consent and that:

  • This is an insurance-billed service. Depending on your plan, you may owe a monthly copay or coinsurance. Our team is happy to help you check your coverage.
  • You can only be enrolled with one provider at a time. You are not enrolled in a similar care coordination program with another provider, and you’ll let us know if that changes.
  • It’s your choice. You can stop at any time, and stopping won’t affect the rest of your care at Journey Clinical.

Your consent stays in effect until you withdraw it. You can see the date you agreed, and withdraw at any time, from the Consents page of your account in the patient portal.
